Book Your Appointment

New Patient Forms
Download our new patient form to fill out before your visit, then email it to us or bring it with you.
Payment Options
We accept all health fund cards using HICAPS, as well as Visa, MasterCard and American Express. We also accept Medicare child benefits scheme and Veteran’s affairs.

Getting Here and Parking
We are easily accessible by bus and a 10 minute walk from St Leonards Station. Parking is available at Coles, Hume Street and Holterman Street offering 2 hours free. We also offer free parking in the building for the duration of your appointment (please call us for directions).
